This roundtable discussion was supported by Neuraxpharm
In this expert roundtable, leading paediatric epilepsy specialists Prof. Angela Kaindl (Berlin), Prof. Stéphane Auvin (Paris), and Prof. Federico Vigevano (Rome) discuss the practical management of prolonged convulsive seizures in children, focusing on early intervention, rescue treatment strategies, and care delivered in the community setting.
The panel examines how prolonged seizures and status epilepticus should be defined in everyday clinical practice, and why fixed time thresholds do not necessarily apply to every patient. They explore how risk varies according to age, epilepsy syndrome, underlying aetiology, and clinical history, with particular emphasis on higher-risk groups such as children with Dravet syndrome and other developmental and epileptic encephalopathies, where prompt treatment may be critical.
Topics covered include:
-
When and for whom rescue medication should be prescribed
-
Practical comparisons between buccal midazolam and rectal diazepam
-
The importance of early treatment to maximise effectiveness and reduce escalation of care
-
Developing individualised seizure action plans
-
Educating families, schools, and caregivers to enable safe and timely administration
-
Addressing barriers such as fear, stigma, and limited training within community environments
Blending evidence, clinical experience, and real-world insight, this session provides practical guidance to support clinicians in optimising acute seizure management while improving outcomes and quality of life for children and their families.
