Abstract:


Dementia is one of the most significant neurological and public health challenges facing Europe. Characterised by progressive cognitive  decline that interferes with daily functioning and independence, dementia affects millions of individuals and places substantial burdens on healthcare systems, caregivers, and society. As populations continue to age, the prevalence of dementia is expected to rise considerably over the coming decades, making prevention, early diagnosis, and effective treatment major healthcare priorities.

Historically regarded as an inevitable consequence of ageing, dementia is now recognised as a complex group of disorders driven by  specific biological processes that begin years or even decades before symptoms emerge. Alzheimer’s disease remains the most common cause of dementia, but vascular dementia, dementia with Lewy bodies, frontotemporal dementia, and mixed dementias contribute significantly to the overall disease burden.Advances in neuroscience have revealed that protein misfolding, neuroinflammation, vascular dysfunction, synaptic loss, mitochondrial impairment, and neuronal degeneration all play important roles in disease progression.

At the same time, growing evidence suggests that many factors influencing dementia risk are potentially modifiable. Cardiovascular health, sleep quality, physical activity, social engagement, hearing loss, education, and mental health may all influence long-term cognitive outcomes. This evolving understanding has shifted attention towards prevention and the promotion of brain health across the lifespan.

This article explores the biological basis of dementia, examines the mechanisms underlying neurodegeneration, reviews the factors associated with cognitive decline, and discusses emerging developments in diagnosis and treatment. Particular attention is given to cognitive reserve, sleep, neuroinflammation, behavioural symptoms, biomarkers, and disease-modifying therapies. Understanding how dementia develops is becoming increasingly important as Europe enters a new era of earlier diagnosis, personalised medicine, and preventative brain healthcare.
